Clare Stephenson is a Clinical Nurse Consultant on the Acute Care of the Elderly ward at Canberra Hospital and a PhD candidate at the University of Canberra with the Centre for Ageing Research and Translation. Her PhD research focuses on dementia‑specific home‑based rehabilitation and centres on the design, implementation, and evaluation of a co‑designed pilot trial aimed at improving post‑diagnostic support, functional and wellbeing outcomes for people with dementia living in the community. Clare’s research and clinical interests align in addressing functional decline during hospitalisation and strengthening transitions of care between acute services and aged care, with a strong focus on enhancing post‑diagnostic care pathways and optimising wellbeing for people living with dementia.
