Supporting Hospital-to-Home Transitions for Mandarin-Speaking Older Adults and Carers: Evaluation of Co-Designed Multimedia Resources

Transitions from hospital to home are a recognised point of risk for older adults and carers, particularly for people from culturally and linguistically diverse communities who may face language barriers and limited familiarity with healthcare systems. We evaluated co-designed resources—a booklet and videos—aimed at supporting Mandarin-speaking older adults and carers transitioning from hospital to home.
A mixed‑methods evaluation was conducted with Mandarin‑speaking older adults and carers who reviewed the resources. Survey data were collected from 16 participants using 11 Likert-scale items and three open-ended questions assessing the acceptability of the resources. In addition, one interpreter supported focus group with 12 participants and one individual interview were conducted to explore the acceptability of the resources in detail and areas for improvement. Quantitative data were analysed descriptively; qualitative data were analysed thematically.
Over 93% of survey respondents agreed or strongly agreed that the resources were relevant and confidence-building, while 87% indicated they would recommend the resources to others. Qualitative findings revealed that the resources provided practical guidance on who to contact, what steps to take, and how to navigate health systems following discharge. Participants emphasised involvement of family carers, early discharge preparation, and the distribution of resources through hospitals, general practices, and aged care organisations. Hardcopy resources were preferred due to limited digital access for some older adults.
The co-designed resources were perceived as acceptable and valuable in supporting hospital‑to‑home transitions. These findings suggest that culturally responsive resources can enhance continuity and confidence for Mandarin‑speaking older adults and their carers.
