Presentation Format
A person-centred and rights-based approach to care is fundamental to ensuring the best quality of life for older people, and is now legislated in the new Aged Care Act. However, truly embedding person-centred and rights-based care requires change at system, organisation and individual levels at a time when there is already significant pressure and change fatigue. Using case studies from ARIIA’s work with aged care providers, the presentation will unpack the barriers and enablers of implementing change projects in aged care, with a particular focus on shifting from traditional models of care to person-centred models of care. It will explore how a combination of inclusive co-design, contextualised evidence, and thoughtful change management can ensure aged care projects are solving the right problem, in the right way, with the right people at the centre.
