As Australia’s ageing population grows, the complexity of health service delivery within aged care continues to increase, particularly for individuals living with neurodegenerative conditions, multimorbidity, and polypharmacy. Oral health is a critical yet often overlooked component of overall health, directly influencing nutrition, swallowing safety, communication, and quality of life. Despite this, oral health remains insufficiently integrated into routine aged care practice and multidisciplinary care models. This presentation explores a community-based model of oral health service delivery through the Smart Smiles Initiative – Healthy Smiles, Healthy Futures, which aims to embed preventative, patient-centred oral health care within aged care settings. The initiative focuses on early intervention, minimally invasive clinical care, and workforce education, with particular attention to residents living with conditions such as dementia, Parkinson’s disease, and Motor Neurone Disease. Drawing on clinical implementation and education programs, this model demonstrates how oral health professionals—including Oral Health Therapists, dental prosthetists, and dentists—can be effectively integrated into multidisciplinary teams. Key components include on-site oral health assessments, denture management, use of preventative therapies such as silver diamine fluoride, and targeted education for nursing and care staff. Importantly, the model highlights the relationship between oral health and dysphagia management, emphasising the role of oral hygiene in reducing aspiration pneumonia risk and improving nutritional outcomes. Early findings indicate improved staff confidence, enhanced oral hygiene practices, and stronger engagement with families. This presentation advocates for the integration of oral health into aged care service delivery and highlights the potential for scalable, preventative models to improve health outcomes and quality of life for older Australians.
