Australia’s aged care system is at a critical juncture, requiring a fundamental shift in how care is designed, delivered, and experienced. Living Well Together (LWT) is a workforce-enabled, person-centred model of residential aged care that challenges task focused care practices and supports a strengths-based, relationship-centred approach to ageing well. Implemented across BaptistCare residential aged care homes in Victoria and Tasmania, LWT is informed by Montessori principles and aims to enhance residents’ autonomy, dignity, independence, and quality of life, particularly for people living with dementia. This presentation shares key implementation and evaluation learnings from a mixed methods study examining how LWT has reshaped service delivery and workforce practice. The model is embedded through meaningful engagement between care staff and residents, supported by targeted workforce development, leadership commitment, interdisciplinary collaboration, and environmental redesign. Findings highlight the importance of frontline champions, aligned leadership, and collaboration between care and lifestyle teams in sustaining practice change. Environmental enhancements, such as improved wayfinding, personalised spaces, and accessible food options, further supported residents’ sense of familiarity, safety, and belonging. Pre- and post- implementation surveys, alongside interviews with residents, families, and staff, demonstrate perceived improvements in quality of life, engagement, and satisfaction with care, as well as shifts in staff confidence and practice. This presentation will critically reflect on practical lessons for scaling and sustaining person-centred reform, positioning Living Well Together as an example of how workforce and service redesign can contribute to a more positive, enabling vision for ageing in Australia.
