Nearly half of older Australians are living with pre-frailty, yet most only encounter aged care support once decline is well established. This creates a critical gap between early prevention and the delivery of funded services under Support at Home. During this time, many experience worsening function, reduced confidence, and increasing reliance on care, or receive support that maintains dependence rather than builds capacity. The result is avoidable escalation of need, increased system costs, and missed opportunities to live well for longer. LiveUp and Keep Able represent digital platforms designed to bridge this divide. LiveUp supports older people—particularly those experiencing early functional changes, including pre frailty— to live and learn through accessible, evidence-based information, self-assessment tools, and practical strategies across intrinsic capacity domains such as mobility, cognition, and social connection. It enables early action, builds capacity, and supports consideration of assistive technology and home modifications before crisis points. Keep Able extends this approach into service delivery by equipping providers, care coordinators, and support workers with the knowledge and tools to embed wellness and reablement within Support at Home. It translates policy into practice, supporting goal-oriented care planning, “doing with not for,” and consistent reablement approaches across the workforce. This presentation will explore the value add of these platforms to the aged care sector — from prevention to care, through bridging waitlists, enhancing consumer preparedness, and enabling providers to deliver outcomes-focused services. We will demonstrate how this approach supports independence, strengthens workforce capability, and contributes to a more sustainable aged care system.
