Background: Older Australians make up a significant proportion of the population in small rural communities. Helping them age well is crucial for their health and wellbeing and for the sustainability of their communities. An age-friendly guide to ageing well (the guide) was developed by a team of older people and health workers across northeast Victoria to inform actions that can prevent, slow, or even reverse common age-related difficulties in four key areas—what matters, medications, mobility, and mental wellbeing. Purpose: This project adopted a collective impact approach with older residents and community organisations in four rural Victorian towns to implement the guide to improve health literacy. Method: A mixed-methods approach was used to evaluate outcomes, collecting survey data and conducting individual and small-group interviews. Data were analysed using Nutbeam’s (2000) three levels of health literacy: functional, interactive, and critical. Findings: The evaluation findings confirm that An Age-Friendly Guide for Living Well is highly effective in supporting health literacy among older people. Its plain language and accessible format made it easy to use, and participants across all sites found it valuable for self-reflection on their health and wellbeing and for preparing for discussions with health professionals. Using the guide gave older people a greater sense of control and confidence in managing their health, an important outcome in rural areas where access to services is limited and the relationship between patients and providers is often ongoing and personal. Conclusion Older people can improve their health literacy when the information they receive is codesigned with older people and supports their agency.
