Background: High‑quality residential aged care (RAC) is underpinned by person‑centred, strengths‑based care, meaningful engagement, enabling environments and dignity of risk. Although these principles are mandated in Australia, their implementation in everyday practice remains inconsistent. Workforce capacity to embed them is shaped by influences across multiple levels of the socio‑ecological model (SEM), yet many interventions fail to address this complexity, limiting sustainability. Aim: To co‑develop an organisational implementation plan for the Continuity of Carer–Supporting Engagement (COC:SE) Program within a national aged care provider in Australia, guided by the SEM and the exploration and preparation stages of implementation science, to strengthen workforce capability and support sustained practice change. Methods: Mixed‑methods study was conducted with a large national provider operating 78 RAC homes. Data sources included eight co‑design workshops with regional and national staff, semi‑structured interviews with staff (n=59) and residents (n=25), environmental assessments (n=10 homes), and document and process reviews across education, communication, and lifestyle programs. Results: Six key implementation focus areas with accompanying operational strategies were identified: training/education; environment; communication/marketing; lifestyle program integration; employee induction; and leadership. The co‑developed implementation plan was pilot‑tested in five homes across Victoria, Western Australia and Queensland. Preliminary findings indicated strong acceptability and feasibility; however, barriers emerged necessitating refinement of delivery, leadership, champions (drivers of change/implementation) and communication strategies. Implications: Findings are informing a staged national rollout guided by an equity lens, supporting adaptability across diverse contexts and strengthening implementation capability to embed and sustain person‑centred, strengths‑based care in RAC.
